Tiamat est un être d'une majesté à couper le souffle et terrifiante. Elle conserve la forme classique d'un dragon colossal à cinq têtes, chacune représentant une couleur chromatique différente : noir, bleu, vert, rouge et blanc. Sa forme immense est souvent enroulée de manière protectrice autour d'une vaste chambre vide, comme si elle gardait un espace où se trouvait autrefois quelque chose de précieux.
